My problem is that I probably doesn't have full version of Quake or
I doesn't understand what you mean with 'cut scenes'. This is something
like video + audio sequences at begin and between episodes?

You can even disable emulating of vibrato (tremolo isn't emulated) in
ultra driver - check change log how can be this done...

Can you send me (directly) some small .wav file which have this symptom?
Looks like these .wav files have a little different header than others and
aplay plays these files as raw files.
