Forgive me, I'm not intimately familiar with the Extreme.  I've only got a
Classic and a PnP.

Is the Extreme a Plug and Play card?  If so, have you used pnpdump and isapnp
to configure the base address before trying to install the Alsa driver?

If it is not Plug and Play, have you checked the jumpers?  Are you sure of
what your base address is?
